Benefits

At Aquilent, one of our goals is to offer comprehensive benefits that provide convenience, reliability, and security to our employees and their families. To follow is a summary of our benefits:

Health Plan

Aquilent employees and their families may elect medical coverage and a prescription plan through Blue Cross Blue Shield. Employees may elect coverage under an HMO or PPO option. Under the HMO, employees have the choice of seeking care from a doctor, hospital or other healthcare provider who participates in the HMO. The co-payment is $10 per office visit to their primary care physician (PCP). With a referral from their PCP, they may seek care from a specialist, which has a co-payment of $20 per office visit. Under the PPO, employees have the choice of seeking care from a doctor, hospital or other healthcare provider who participates in the PPO and paying $10-15 per regular office visit or seeking care from a non-participating provider and with annual deductible and co-insurance limits applying.

Dental Plan

Aquilent offers dental coverage for our employees and their families through Guardian. The dental benefit covers 100% of qualified preventive dental needs with no deductible.

Vision Plan

Aquilent offers vision coverage for our employees and their families through Guardian. Guardian uses the Vision Service Plan (VSP) network of providers. If seeking care from a participating provider, the vision benefit covers 100% of annual eye exams and lenses (contact or eyeglass) as well as and eyeglass frames every 24 months, after a $10 copay.

Life Insurance/Accidental Death & Dismemberment

The life insurance and accidental death plan offers financial protection for our employees' beneficiaries in the event of an employee's death. The plan offers coverage of two times an employee's annual salary up to $500,000 for both the company paid life insurance and the accidental death insurance. Under the accidental death and dismemberment policy, in the case of serious injury, the benefit paid is dependent upon the loss.

Supplemental Life Insurance

This program allows employees to increase their life insurance coverage in $10,000 increments, up to $500,000. The employee may also purchase life insurance coverage for his/her spouse and children.

Supplemental Accidental Death and Dismemberment (AD&D)

This program allows employees to increase their AD&D coverage in $50,000 increments, up to $500,000. The employee may also purchase AD&D coverage for his/her spouse and children.

Short and Long Term Disability

Aquilent offers company paid short and long-term disability coverage for its employees. Short-term coverage, after a 7-day elimination period, replaces 70% of the employee's weekly salary, up to $2,500 weekly, for the first 13 weeks. Long-term coverage replaces 60% of the employee's monthly salary, up to $5,000 month.

Flexible Spending Accounts

This plan allows employees to set aside pre-tax dollars to pay for qualified health care expenses that are not covered by health insurance and for eligible child-care expenses.

Transportation Benefit Program

The transportation benefit allows those employees using public transportation to get to their worksite to pay for their fares using pre-tax dollars (up to $100 per month), and those employees who pay to park their vehicles at their worksite or at a public transit site, to be reimbursed from pre-tax dollars (up to $205 per month).

Retirement Investment Program - 401(k)

One of the most important Aquilent benefits is our 401(k) program, a long term savings plan designed to help employees with their retirement planning and other long-term saving needs. Employees can defer up to 25% of their salaries before taxes, to the IRS annual maximum ($15,000 in 2006). Savings also grow via the company's matching contribution. Currently, the company will match $.50 on each $1 of employee's contribution, not to exceed 3% of the employee's compensation.

Educational Assistance

Aquilent encourages personal and professional development through training and formal education. Aquilent offers a tuition reimbursement program which covers tuition, textbook costs, and laboratory fees.

Paid Time Off

Aquilent employees earn between 15 and 20 days of Paid Time Off (PTO) based upon years of applicable professional experience. Over an employee's tenure with Aquilent they will gain more professional experience and will be rewarded accordingly with additional PTO. Additionally, Aquilent provides 10 paid holidays per year.
